💊 Severe pain management – Hydromorphone is an opioid pain medicine used for pain severe enough to require opioid treatment when other pain medicines have not worked well enough or cannot be tolerated. Extended-release hydromorphone is used for severe pain requiring around-the-clock, long-term pain relief in opioid-tolerant patients.

⏳ Extended-release support – If this is an 18 mg controlled-release product, it is intended to release medication over time for ongoing pain control, not for casual, occasional, or “as-needed” pain use. Hydromorphone ER products are specifically labeled for severe persistent pain in opioid-tolerant patients and are not PRN/as-needed analgesics.

Important 18 mg warning

⚠️ 18 mg hydromorphone is a strong opioid dose. A Canadian controlled-release hydromorphone monograph warns that a single dose greater than 12 mg, or total daily dose greater than 24 mg, may cause fatal respiratory depression in patients not tolerant to similar opioid doses.

Major risks

🚫 Do not crush, chew, dissolve, snort, or inject — extended-release hydromorphone must be swallowed whole. Tampering with it can release too much hydromorphone at once and may cause overdose or death.

🍺 Do not mix with alcohol, Xanax/benzodiazepines, sleep meds, gabapentin/pregabalin, or other depressants — these combinations can cause severe drowsiness, slowed breathing, coma, or death.

⚠️ Addiction, dependence, overdose risk – Hydromorphone can cause addiction, abuse, misuse, overdose, and death, even when prescribed. Higher opioid doses increase overdose risk.

Use only exactly as prescribed. Store locked away, never share it, and ask a doctor or pharmacist about naloxone for overdose emergencies.
